Accurate traveler details are required to complete your booking. This form collects everything your specialist needs to finalize your trip.
What we collect
To book your Disney vacation we need verified details for every traveler in your party. Depending on your trip type, this typically includes:
- Legal name (as it appears on government-issued ID or passport)
- Date of birth
- Passport number and expiration date (required for Disney Cruise Line and international travel)
- Dietary restrictions and accessibility needs
- Emergency contact information
All information is stored securely in your Mouseways client file and used only for the purpose of completing your booking.
Complete your guest profile
The guest information form is accessed through your Mouseways client portal (Trip Hub). Your specialist will send you a direct link when your booking is ready to be finalized.
